When it comes to deciding how you or someone you care for will spend their later years in life, there are certainly many questions to consider. Questions like “How can I stay active?” or “How can I continue to do something meaningful?” Or you may be wondering, “How will I cope with this life-changing decision?” The different levels of senior care options at Mission Health—from Independent Living to Assisted Living to Skilled Nursing Care—all offer a range of activities, supports, and resources that will help put your mind at ease.
On the day you are admitted, you will be seen by a therapist who will assess your condition. After this evaluation, your therapy team will develop an individualized care plan for you, which will determine the frequency and duration of your therapy. We offer therapy services seven days a week along with group programs and individual activities to suit your interests.
Providing patient-centered care is something we’re focused on delivering. That’s why we take the time to listen, so we can understand your needs and preferences. Then we establish clinical, dining, and program support that’s tailored to your specific needs and fulfills them in a meaningful way.
At our community, we do all we can to ensure you stay connected with your family and friends. At the same time, we encourage the people in your life to develop relationships with our staff to help facilitate ongoing communication. Consistent communication creates a comfortable rapport, where any and all questions are welcome—because we’re here to answer them.
You and your family members are encouraged to participate in progress discussions that are held periodically throughout your stay.
Some examples of what you might discuss include upcoming plans for your discharge (if you’re a short-stay rehab patient) or a change in your existing course of care.
